There are a number of service providers which specialise in providing assistance to people with disabilities seeking employment.
Services may be able to assist by:

Assisting you in working out any work place adjustments you may need

Providing assistance in decisions such as whether to disclose your disability

DisabilityWorks is a major service provider in assisting job seekers with disabilities.
UniSA Careers Advisers can provide assistance in the application process.

When and how you disclose your condition is entirely your decision. You may feel that the employer does not need to know.
